Patient Forms

Upon entering our office, our receptionist will  welcome you as a member of our practice.  We will request that you complete our  patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition. Feel free to print patient forms off of our website and fill them out before you come in to save time.

Consultation

Next, you will have a consultation with Dr. Ty Dahodwala or Dr. Heidi Williams to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for us to learn more about you, your condition, and  expectations to determine how we can help you meet your goals.

Examination

After your consultation, we will perform a complete physical examination that concentrates on your specific condition. We may test range of motion, reflexes, muscle strength, sensitivity to pain as well as standard neurological, orthopedic, postural, and physical tests to fully diagnose your problem. Our goal is to get enough information to design the safest, most effective course of care and determine if we need to refer you to another healthcare provider.  We make an effort to explain what we are doing to address your fears and cater our examination as best as possible to your level of pain tolerance.

Initial Treatment

At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage.  Treatment is interactive, so  you can express concerns about the different styles of treatment  anytime. Most of our patients notice marked improvement after the first appointment. Results depend on numerous factors, including, but not limited to: age, physical fitness, and severity of your injury.
